Medical Release and Safety Acknowledgments

By marking agree, as parent and/or guardian, I authorize First Unitarian Religious Education program staff or volunteers in the case of medical emergency to give first aid or take my child(ren) to the nearest medical facility, and I authorize the treatment of my child(ren) by a qualified and licensed medical professional in the event of a medical emergency which may endanger my child's life or cause disfigurement, physical impairment or undue discomfort if delayed. This authority is granted only if I am not reached after a reasonable effort has been made to contact me.

Next Meeting - 9/26/25

Open Spark Youth (OSY) is a group that creates a safe and brave space for friendship, creativity, learning, and action. OSY is a ministry of the First Unitarian Church of Chicago. The group is open to all youth grades 8-12 (or equivalent), regardless of faith, tradition, affiliation, or lack thereof. We believe that our differences make us stronger.

This form is where parents/guardians must register their youth the first time they plan to attend an Open Spark event.
